What you can do, if the third button works under gpm, is to run
gpm with the -R option; this repeats incoming mouse stuff to
/dev/gpmdata, which you then use in XF86Config as your mouse
device.  You tell X that the mouse is a mouse systems mouse, as
I recall, and the third button then works in X.
